Explanation of Benefits

Your explanation of benefits (EOB) from Network Health explains what services were paid for and how much you may owe. It is important to compare your Explanation of Benefits with any invoices you receive from services.
 

Forms

The Personal Health Information Consent form allows a specified person (a spouse, relative, friend, advocate, attorney, doctor or someone else) to call to discuss your coverage and plan information if it’s ever needed.
The Appointment of Representative form names a relative, friend, advocate, doctor or someone else to act on your behalf for an appeal or complaint.
 

Coordination of Benefits

Staff who are enrolled in a Froedtert Health medical plan must complete a Coordination of Benefits form on an annual basis. If Network Health does not receive a completed form, claims may be denied until the form is received.

Completed forms can be submitted via direction on the form.
