The Personal Health Information Consent form allows a specified person to call to discuss your coverage and plan information when needed.
The Appointment of Representative form names someone to act on your behalf for an appeal or complaint.
The Coordination of Benefits form is required on an annual basis for staff enrolled in a Froedtert Health medical plan. Claims may be denied until the form is received.
